These referral forms are available to both Doctors and the public. A referral for the Thoracic and Sleep Group (QLD) is only valid once completed by a medical professional (GP or Specialist).
From November 2018, there will be a number of changes to procedures on the Medicare Benefits Schedule (MBS) for Sleep medicine. Further (more detailed) information about the changes will also be available on the MBS Online website: https://www.mbsonline.gov.au/
Once your referral has been completed by a medical professional your booking can be made by contacting the Thoracic and Sleep Group Queensland (TSGQ) on (07) 3870 1120 or 1800 119 446.
